All other members wanting a paper copy will need to request one by visiting MyHumana or calling 800-4HUMANA (800-448-6262). An electronic copy of form 1095-B will be posted to myhumana.com for all members whom Humana files a 1095-B with the IRS and states. All members will have the ability to download and print the form themselves. Form 1095-B will be made available to you no later than the annual deadline set by the IRS.
